РОЛЬ ТРЕТІХ МОЛЯРІВ У РОЗВИТКУ СКУПЧЕНОСТІ ЗУБІВ НА НИЖНІЙ ЩЕЛЕПІ: АНАЛІЗ ДОСТУПНИХ ДОКАЗІВ ЗА ДАНИМИ СИСТЕМАТИЧНИХ ОГЛЯДІВ

Анотація. Вступ. Враховуючи відмінності результатів окремих наукових публікацій щодо потенційних взаємозв’язків між параметрами наявності, положення та напрямку прорізування третіх молярів і ймовірністю розвитку скупченості передніх зубів нижньої щелепи, доцільним є проведення аналізу доказів, сформульованих на основі даних систематичних оглядів, як агрегатів опрацьованих результатів попередньо проведених досліджень згідно відповідно сформульованих та строго дотриманих критеріїв аналізу. Мета. Оцінити вплив третіх молярів на ризик розвитку скупченості зубів на нижній щелепі згідно даних, акумульованих у доступних систематичних оглядах. Методи. Первинний пошук цільових наукових джерел, пов’язаних із сформульованою метою дослідження, проводився у базі даних PubMed (https://pubmed.ncbi.nlm.nih.gov/) з використанням ключових слів “tooth crowding” та “third molar” та залученням фільтрів “Systematic review” та “Meta-Analysis”. Результати. Докази для констатації причино-наслідкового зв’язку між впливом третіх молярів та розвитком скупченості зубів, відображені у систематичних оглядах, є недостатніми. Треті моляри не є основною причиною розвитку скупченості передніх зубів нижньої щелепи, а тільки одним з факторів, які приймають участь у формування даної патології зубного ряду; при цьому клінічна вираженість ознак скупченості зубів нижньої щелепи потенційно може бути вищою у пацієнтів з наявними третіми молярами. Висновки. Попри те, що результати окремих систематичних оглядів та метааналізів вказують на наявність потенційного зв’язку між ймовірністю розвитку патології скупченості зубів та наявністю, положенням, напрямком прорізування та стадією розвитку нижніх третіх молярів, докази такого зв’язку характеризуються низьким рівнем якості, і він не може бути категоризований у якості причинно-наслідкового. У доступних систематичних оглядах не було доведено впливу третіх молярів на розвиток рецидиву скупченості зубів після проведеного ортодонтичного лікування, що виключає необхідність реалізації процедури їх екстракції як обов’язкової специфічної маніпуляції з метою попередження розвитку ортодонтичного порушення.
